We understand that your time is precious, and no one enjoys filling out medical forms at their new patient appointment. For this reason, we have made our new patient documents available here for you to print out and fill out before your first appointment.
When you arrive at your first appointment, we will review your documents, evaluate your medical history, and see what brings you to our office. We love seeing your goals, and we are thrilled you chose Chappaqua Dentistry in Westchester, NY, to care for your oral health (and hopefully your loved ones, too).
Below, you will find links to our new patient forms. If you have any questions or trouble accessing these forms, please call our amazing front desk team, who will be happy to help. If you’d rather fill out the forms in the office upon arrival, that’s fine, too.
To provide you with the best dental care possible, the more we know about your medical history and dental concerns, the better. We must have an in-depth understanding of your past medical history to provide you with safe and effective dental care.
You’d be amazed at how closely tied oral health and general health are, and it’s important we know what medical conditions you have and what medications you take. We want to care for your whole health, not just your oral health, and to do that, we have to know your full medical history.
Your first appointment will take some time as we are thorough and want to learn as much about you as possible. Your new patient comprehensive exam will involve x-rays, oral cancer screening, evaluation of your current dental situation, and some time with Dr. Brisman to review his findings.
